REGISTRATION
All registrations require a $10.00 deposit fee mailed to the following address.
If you do not wish to electronically submit registration, mail completed regsitation forms, individual information forms, and on (1) check made out to Camp Lakeview for the total non-refundable deposit of $10.00 for each participant. Please do not use staples or tape when enclosing your check with your forms.

The group leader is required to complete the individual information form before arriving at the event. Your congregation is responsible for the selection of adult leaders, their conduct with youth, and an understanding between adult leaders and parents of youth regarding the handling of possible emergencies. In addition to this form, you may wish to obtain medical release signatures from the parents of participating youth. All registrants and families are address to the Awestruck database and will receive information on opportunities to be involved with Awestruck's ministry. If there is any family on the list who should not be given opportunities for involvement, please write DNC (do not contact) by their name.
